Physical Examination

  • Students registering for Athletics & Activities must have a current physical examination form submitted for the Athletics Trainer’s review before the student is authorized to participate in practice or tryouts.
  • Physicals are valid for 365 days from the date of the exam.
  • Download the physical form by clicking the your school's Team Registration icon above and complete a family account and follow download instructions.
